Breathwork encompasses deliberate control of breathing rate, depth, and rhythm to influence the autonomic nervous system. The most evidence-backed form is slow-paced breathing at approximately 4.5–6 breaths per minute, roughly five to six seconds per inhale and exhale, which aligns breathing with the heart’s natural resonance frequency and maximizes respiratory sinus arrhythmia (RSA), the synchronization of heart rate and respiration.

A 2022 systematic review and meta-analysis of 223 studies on voluntary slow breathing confirmed significant increases in vagally-mediated heart rate variability (HRV), specifically RMSSD and SDNN, at all measured time points: during sessions, immediately after, and following multi-session training protocols. HRV is a measure of parasympathetic nervous system activity and autonomic flexibility, and higher resting HRV is independently associated with lower risk of cardiovascular disease, hypertension, depression, and all-cause mortality. A 2024 meta-analysis of 31 studies confirmed immediate reductions in systolic blood pressure (SMD = −0.45) from slow-paced breathing sessions.

For stress and mental health, a 2023 meta-analysis of randomized controlled trials found that breathwork interventions significantly reduced self-reported stress compared to non-breathwork controls (g = −0.35). Benefits were observed even from single sessions, representing an unusually accessible stress-reduction tool. A narrative review identified both sustained (resting HRV improvements after multi-week training) and acute (immediate BP and anxiety reduction) effects.

The mechanism runs through vagal efferent stimulation: slow, deep diaphragmatic breathing activates stretch receptors in the lungs and engages the baroreflex arc, increasing parasympathetic outflow and reducing sympathetic tone. This directly counters the chronic sympathetic activation associated with modern stress, disrupted sleep, and sedentary behavior. Optimal protocol: 5–15 minutes daily at 4.5–6 breaths per minute, ideally practiced consistently at the same time each day for cumulative resting HRV improvements.
